The “No One Needs to Know” Patient
What Annie Says:
- “I want to look better, but I don't want anyone to know I did anything.”
- “My lips are disappearing, but I don't want duck lips.”
- “My lipstick keeps bleeding into these little lines.”
- “I’m scared I won’t look like myself.”
- “Can we start really small?”
What May Be Happening:
With age, the lips can gradually lose volume, shape and definition. The skin around the mouth also becomes thinner, while years of smiling, talking and puckering can make fine lines more noticeable.
What We May Consider:
Annie is not looking for a dramatic change. She wants to look refreshed without looking treated. For Annie, we might consider a soft, lightweight filler that moves naturally and blends smoothly into delicate tissue with the least amount of swelling and bruising. The goal is to soften fine lines or restore subtle lip definition without creating a lot of projection or noticeable fullness.
For Annie, subtle does not simply mean using less filler. It means choosing the right texture, placement and amount for her anatomy.

2. "Sultry" Sally
The “More Is More” Patient
What Sally Says:
- “I want lips!”
- “I brought pictures.”
- “I want them fuller, especially on top.”
- “I don’t mind if people notice.”

What May Be Happening:
Sally may naturally have smaller lips, less definition or uneven proportions between her upper and lower lip. She may also have lost some of her natural lip volume over time. Or she may simply prefer a fuller, more noticeable look.
What We May Consider:
Sally is not asking for an undetectable change. She wants more volume, more shape and a result she can actually see. For her, we may consider a filler designed to build fullness and create more projection while still feeling soft. All hyaluronic acid fillers attract water, but some attract more than others. A more water-loving filler can help give Sally the juicy, hydrated and pillowy look she wants.
Sally knows what she wants. Our job is to create the fullness she loves while keeping her lips beautifully shaped and balanced with the rest of her face.

3. "Bring-’Em-Back" Betty
The “Restore, Don’t Reinvent” Patient
What Betty Says:
- “I feel like all of a sudden I look old.”
- “I look tired even when I’m not.”
- “My face looks heavier down here.”
- “I really notice the sagging in pictures, especially when I’m looking down.”
- “I keep pulling everything back like this, but I don’t want surgery.”
What May Be Happening:
Facial aging is not only about wrinkles. Over time, we lose collagen, fat and some of the underlying bone that once supported the face. As that support decreases, the cheeks may look flatter, folds can become deeper and tissue may begin to sit lower.
These changes can make the lower face look heavier and the jawline less defined. The natural size and shape of the chin can also affect how balanced the lower face looks, even though Betty may never think to mention her chin during a consultation.
What We May Consider:
Betty may not know why her face looks heavier or why she suddenly looks older. She only knows that the face looking back at her does not match the way she still feels. She does not want a different face. She wants to look more like herself again.
For Betty, we may consider a firmer filler that can be placed deeply and hold its shape. Instead of acting like a surface-level plumper, this type of filler works more like an underlying foundation.
It may be used to rebuild support through the cheeks, soften the appearance of folds or add definition through the chin and jawline. In the right patient, carefully placed filler can create a more supported, lifted appearance and improve the balance of the face without surgery.
Betty does not need every line filled. She needs an injector who understands where support has been lost and where restoring a little shape could make the most meaningful difference.
